Agonistas del receptor de trombopoyetina en la trombocitopenia inmunitaria primaria crónica: Eficacia del eltrombopag y su papel como alternativa a la esplenectomía

Antecedentes: La trombocitopenia inmunitaria primaria (PTI) es un trastorno autoinmune que se cronifica en el 60 %-70 % de los adultos. Los tratamientos de primera línea, como los corticosteroides, se asocian a altas tasas de recaída y efectos adversos graves con el uso prolongado. Los agonistas del receptor de trombopoyetina (ARTPo), como el eltrombopag (oral), surgieron como una innovación terapéutica con un mecanismo dual trombopoyético e inmunomodulador. El objetivo de la revisión fue evaluar la eficacia del eltrombopag para lograr una respuesta plaquetaria sostenida y su papel como alternativa a la esplenectomía.
Métodos: Se realizó una revisión integrativa siguiendo el protocolo PRISMA 2020. Se llevó a cabo una búsqueda sistemática en cuatro bases de datos electrónicas, abarcando la evidencia desde enero de 2020 hasta marzo de 2026. Los criterios de elegibilidad (marco PICO) se centraron en adultos con PTI crónica/persistente (duración ≥ 3 meses, plaquetas < 30 × 10⁹/L), comparando ARTPo frente a placebo, tratamiento estándar o esplenectomía. Los desenlaces incluyeron la respuesta plaquetaria (≥ 50 × 10⁹/L), la remisión sostenida sin tratamiento (RSST) y la tasa de esplenectomía.
Resultados: Se incluyeron 58 estudios con más de 18 500 participantes. El eltrombopag demostró una superioridad estadísticamente significativa frente al placebo para la respuesta plaquetaria (RR = 3,42; P < 0,0001), con tasas de respuesta global que alcanzan hasta el 91 % en estudios de vida real a largo plazo (> 12 meses). Se observó una inducción de RSST en el 25 %-53 % de los pacientes tras la discontinuación gradual. La adopción de los ARTPo se asoció con una reducción en la frecuencia de esplenectomías. El perfil de seguridad mostró disfunción hepática (8,2 %-22,4 %) y eventos trombóticos (2,2 %-3,2 %), sin un aumento significativo respecto al placebo.
Conclusión: El eltrombopag logra consistentemente una respuesta plaquetaria sostenida superior al placebo y tiene la capacidad de inducir RSST. Su doble mecanismo (trombopoyético e inmunomodulador) justifica su posición como alternativa médica preferente a la esplenectomía en la PTI crónica refractaria a la primera línea. Las prioridades futuras de investigación incluyen la identificación de biomarcadores de RSST y el desarrollo de análisis económicos en sistemas de salud con bajos recursos.
