Abstract

Introduction: Acute appendicitis is the most frequent abdominal surgical emergency. In complicated forms morbidity rises considerably, and appendiceal peritonitis is one of the main causes of secondary peritonitis in the country, where this burden has scarcely been described in district hospitals. The aim was to describe the frequency and characteristics of complicated acute appendicitis and of appendiceal peritonitis among appendectomies performed at the District Hospital of Hernandarias, Paraguay, during the first quarter of 2023. Materials and methods: Observational, descriptive and retrospective study of the medical records of patients who underwent appendectomy between January and March 2023, with complete records of age, diagnosis and postoperative management. Sex, age, appendicitis classification, type of complication and in-hospital mortality were described using frequencies, without hypothesis testing. Results: Forty-one appendectomies were analysed. Seven cases (17.1%) corresponded to complicated appendicitis and 34 (82.9%) to simple appendicitis. All seven complicated cases presented appendiceal peritonitis: four women and three men; five were aged 20–40 years, one under 20 and one over 40. There were no deaths during hospitalization. Conclusions: In this short series from a district hospital, about one in six patients operated on for appendicitis had complicated disease, always as peritonitis, without mortality. The findings are descriptive, do not allow estimation of population prevalence or assessment of care performance, and support incorporating time to presentation and surgical approach into the operative record to guide prevention.
