Respuesta excepcional al tratamiento con Alectinib en una paciente joven no fumadora con adenocarcinoma pulmonar ALK-positivo: Un reporte de caso

Resumen

Objetivos: Analizar un caso clínico de adenocarcinoma de pulmón de células no pequeñas (CPCNP) en una paciente joven no tabaquista, con enfoque en la influencia del gen ALK y la respuesta al tratamiento dirigido.

Métodos: Se llevó a cabo un estudio de caso de una mujer de 42 años con dolor en la espalda derecha, falta de aire y tos sanguinolenta. Se llevaron a cabo estudios de imagen (TAC), biopsia pulmonar, análisis inmunohistoquímico, estudio molecular del gen EGFR e inmunoexpresión del marcador PD-L1 con anticuerpo 22C3. Se evaluó la presencia del marcador ALK con anticuerpos D5F3 y se inició tratamiento con Alectinib 150 mg.

Resultados: El diagnóstico reveló un adenocarcinoma con células tumorales mostrando baja inmunoexpresión de PD-L1 (TPS entre 1-49 %) y translocación ALK positiva por anticuerpo D5F3. Tras el tratamiento con Alectinib, se logró una remisión casi completa del tumor. La paciente alcanzó una clasificación ECOG 0 y pudo reanudar sus actividades laborales con normalidad, sin necesidad de tratamientos adicionales.

Conclusiones: Este caso destaca la importancia de la identificación de la translocación del gen ALK en pacientes jóvenes y no fumadores con CPCNP. El tratamiento dirigido con Alectinib demostró ser altamente efectivo, logrando una remisión significativa del tumor y una mejora sustancial en la calidad de vida de la paciente, sin efectos adversos graves.
