Injuria renal aguda en unidad de cuidados intensivos

Resumen

Introducción: Los objetivos fueron: 1) Describir el tipo de población afecta a injuria renal aguda (IRA) en el contexto de una hospitalización en unidad de cuidados intensivos; 2) determinar la etiología de la IRA en la población de estudio, describir los tipos de tratamientos recibidos tanto médicos como de terapias de reemplazo renal; 3) cuantificar la mortalidad intrahospitalaria y el grado de recuperación de la función renal en la población superviviente.

Materiales y métodos: Este fue un trabajo observacional prospectivo conducido en un hospital universitario de tercer nivel, desde el 1 de enero al 31 de diciembre de 2018. En este lapso se reclutaron 142 pacientes con una creatininemia superior a 2 mg/dl o aumento del 50% sobre sus valores basales.

Resultados: En nuestra serie notamos una preponderancia masculina (63%), con comorbilidades (89%). Entre ellas, la diabetes e hipertensión arterial fueron las más frecuentes (76%). Encontramos un contingente considerable de IRA prerrenal (61%) que luego evolucionaría a formas más graves relacionadas con sepsis y choque séptico (68%). Los puntajes de gravedad al ingreso y evolutivos se correlacionaron con una mortalidad elevada (77%).

Conclusiones: No pudimos descartar el papel de los diuréticos en la mala evolución.
